Aeroflot Flight 699 was a scheduled flight, operated by Tupolev Tu-154B CCCP-85254, from Moscow Domodedovo Airport to Krasnovodsk Airport (now Turkmenbashi International Airport) that crashed on approach to its destination.
The aircraft made a very heavy landing and broke-up, after a poorly executed approach by an inexperienced co-pilot.
The accident aircraft was a Tupolev Tu-154B-1 operated by Aeroflot, registered as CCCP-85254 (msn 78A254), which was first flown in 1977 and had a total of 1589 flying hours accumulated over 8082 cycles. [1]
The investigation found the aircraft to be serviceable at the time of the crash with no significant faults. The crew came under the spotlight and were found to be seriously lacking in crew resource management and skills to carry out a challenging approach. The Pilot in Charge allowed an inexperienced co-pilot to carry out the approach without close supervision, which resulted in an unstable approach and very heavy landing estimated at 4.8g. The aircraft broke-up and 11 passengers were thrown from the fuselage and killed. [1]
The primary cause of the accident was the poor crew resource management of the Pilot in Charge, who failed to closely monitor the Pilot Flying who had limited experience and skill. [1] [2]

Aeroflot Flight 3352 was a regularly scheduled Aeroflot domestic flight in the Soviet Union from Krasnodar to Novosibirsk, with an intermediate landing in Omsk. While landing at Omsk Airport on Thursday, 11 October 1984, the aircraft crashed into maintenance vehicles on the runway, killing 174 people on board and four on the ground. While a chain of mistakes in airport operations contributed to the accident, its major cause was an air traffic controller falling asleep on duty.
Aeroflot Flight 4225 was a Tupolev Tu-154B-2 on a scheduled domestic passenger flight from Alma-Ata Airport to Simferopol Airport on 8 July 1980. The aircraft had reached an altitude of no more than 500 feet when the airspeed suddenly dropped because of thermal currents it encountered during the climb out. This caused the airplane to stall less than 5 kilometres from the airport, crash and catch fire, killing all 156 passengers and 10 crew on board. Aeroflot Flight 217 was a non-scheduled international passenger flight from Orly Airport in Paris to Sheremetyevo International Airport in Moscow, with a stopover at Shosseynaya Airport in Leningrad. On 13 October 1972, the Ilyushin Il-62 airliner operating the flight crashed on approach to Sheremetyevo, with the loss of all 164 passengers and crew of 10. At the time, it was the world's deadliest civil aviation disaster, until it was surpassed by the Kano air disaster in Nigeria in 1973. As of 2023, the crash of Flight 217 remains the second-deadliest accident involving an Il-62, after LOT Flight 5055, and the second-deadliest on Russian soil, after Aeroflot Flight 3352.

On 3 September 1962 the Aeroflot Tupolev Tu-104 lost control after the airframe started vibrating, resulting in the plane rolling and yawing several times at an altitude of 4,500 meters before crashing. The aircraft crashed into a swamp, some 90 kilometers away from Khabarovsk. At the time, it was the deadliest crash in the history of Soviet aviation.

Seventy of the 94 passengers and crew on board were killed when the plane overran the runway, after the pilot made a bet that he could make an instrument-only approach with curtained cockpit windows. Investigators determined the cause of the accident was pilot negligence.

The 1958 Aeroflot Тu-104 Kanash crash occurred on 17 October 1958 when a Tupolev Tu-104A operated by Aeroflot flying an international route from Beijing to Moscow crashed in bad weather near the town of Kanash, Chuvashia, Soviet Union, four hundred miles east of Moscow, killing all 80 people on board. The flight was carrying high-level diplomatic delegations from numerous Soviet aligned countries such as China, East Germany, and Czechoslovakia. It was just the second fatal accident involving the Tu-104 which had been introduced into Aeroflot's inventory two years earlier, and the deadliest in the airline's history until the crash of Aeroflot Flight 902 in 1962.
Aeroflot Flight 6709 was a Tupolev Tu-154B on a domestic route from Baku to Leningrad on 19 May 1978. While cruising, fuel starvation affected the flow of fuel to the aircraft's three Kuznetsov NK-8 engines, causing the engines to stop. This issue was possibly as a result of poor aircraft design.
