Echopraxia

Echopraxia (also known as echokinesis [1] ) is the involuntary repetition or imitation of another person's actions. [1] Similar to echolalia, the involuntary repetition of sounds and language, it is one of the echophenomena ("automatic imitative actions without explicit awareness"). [1] It has long been recognized as a core feature of Tourette syndrome, [1] and is considered a complex tic, but it also occurs in autism spectrum disorders, [1] [2] schizophrenia and catatonia, [1] [3] aphasia, and disorders involving the startle reflex such as latah. [1] [4] Echopraxia has also been observed in individuals with epilepsy, dementia and autoimmune disorders; [1] the causes of and the link between echopraxia and these disorders is undetermined. [5]

The etymology of the term is from Ancient Greek: "ἠχώ (ēkhō) from ἠχή (ēkhē "sound") and "πρᾶξις (praksis, "action, activity, practice)". [6]

Characteristics

Echopraxia is the involuntary mirroring of an observed action. Imitated actions can range from simple motor tasks such as picking up a phone to violent actions such as hitting another person.[ citation needed ]

Imitative learning and emulation of physical and verbal actions are critical to early development (up to the age of two or three), but when these behaviors become reactions rather than a means for learning, they are considered echophenomena (copying behaviors). [1]

Causes

Echopraxia is a typical symptom of Tourette syndrome but causes are not well elucidated. [1]

One theoretical cause subject to ongoing debate surrounds the role of the mirror neuron system (MNS), a group of neurons in the inferior frontal gyrus (F5 region) of the brain that may influence imitative behaviors, [1] but no widely accepted neural or computational models have been put forward to describe how mirror neuron activity supports cognitive functions such as imitation. [7]

Diagnosis

There is no formal test for diagnosing echopraxia. It is easier to distinguish in individuals over the age of five, because younger children frequently imitate others' actions.

Imitation can be divided into two types: imitative learning and automatic imitation. [1] Imitative learning occurs when a person consciously mimics an observed action in order to learn the mechanism behind that action and perform it themselves. Babies begin copying movements soon after birth; this behavior begins to diminish around the age of three. Before that, it is not possible to diagnose echopraxia, because it is difficult to differentiate between imitative learning and automatic imitation. If the imitative behavior continues beyond infanthood, it may be considered echopraxia. [1]

Echopraxia may be more easily distinguished in older individuals, because their behaviors in relation to prior behaviors can be differentiated. They report feeling an uncontrollable urge to perform an action after seeing it being performed. Automatic behavior is occasionally present in healthy adults (for example, when a person observes someone yawning, he or she may do the same); these behaviors are not considered echopraxia.

Tourette syndrome or Tourette's syndrome is a common neurodevelopmental disorder that begins in childhood or adolescence. It is characterized by multiple movement (motor) tics and at least one vocal (phonic) tic. Common tics are blinking, coughing, throat clearing, sniffing, and facial movements. These are typically preceded by an unwanted urge or sensation in the affected muscles known as a premonitory urge, can sometimes be suppressed temporarily, and characteristically change in location, strength, and frequency. Tourette's is at the more severe end of a spectrum of tic disorders. The tics often go unnoticed by casual observers.

Coprolalia is involuntary swearing or the involuntary utterance of obscene words or socially inappropriate and derogatory remarks. The word comes from the Greek κόπρος, meaning "dung, feces", and λαλιά "speech", from λαλεῖν "to talk".

Imitation is a behavior whereby an individual observes and replicates another's behavior. Imitation is also a form of that leads to the "development of traditions, and ultimately our culture. It allows for the transfer of information between individuals and down generations without the need for genetic inheritance." The word imitation can be applied in many contexts, ranging from animal training to politics. The term generally refers to conscious behavior; subconscious imitation is termed mirroring.

Latah is a condition in which abnormal behaviors result from a person experiencing a sudden shock or other external stressor almost exclusively having been observed in persons from Southeast Asia. When induced, the affected person typically engages in such behaviors as screaming, cursing, dance movements, uncontrollable laughter, mimicry and command obedience. Physical symptoms include an increased heart rate and profuse sweating, but no clear physiological causality beyond the apparent relationship between sudden shock and/or severe emotional stress have been identified. Since no research has emerged indicating whether the behavior is caused by a genetic disorder unique to those of Southeast Asian ancestry, a set of psychosomatic symptoms triggered by Southwest Asian cultural anthropological factors, or another cause not yet hypothesized, the cause has remained undetermined.

Echolalia is the unsolicited repetition of vocalizations made by another person. In its profound form it is automatic and effortless. It is one of the echophenomena, closely related to echopraxia, the automatic repetition of movements made by another person; both are "subsets of imitative behavior" whereby sounds or actions are imitated "without explicit awareness". Echolalia may be an immediate reaction to a stimulus or may be delayed.

The Jumping Frenchmen of Maine were a group of 19th-century lumberjacks who exhibited a rare disorder of unknown origin. The syndrome entails an exaggerated startle reflex which may be described as an uncontrollable "jump"; individuals with this condition can exhibit sudden movements in all parts of the body. Jumping Frenchmen syndrome shares some symptoms with other startle disorders.

A mirror neuron is a neuron that fires both when an organism acts and when the organism observes the same action performed by another. Thus, the neuron "mirrors" the behavior of the other, as though the observer were itself acting. Mirror neurons are not always physiologically distinct from other types of neurons in the brain; their main differentiating factor is their response patterns. By this definition, such neurons have been directly observed in humans and primate species, and in birds.

A stereotypy is a repetitive or ritualistic movement, posture, or utterance. Stereotypies may be simple movements such as body rocking, or complex, such as self-caressing, crossing and uncrossing of legs, and marching in place. They are found especially in people with autism spectrum disorders and visually impaired children, and are also found in intellectual disabilities, tardive dyskinesia and stereotypic movement disorder, yet may also be encountered in neurotypical individuals as well. Studies have shown stereotypies associated with some types of schizophrenia. Frontotemporal dementia is also a common neurological cause of repetitive behaviors and stereotypies. Several causes have been hypothesized for stereotypy, and several treatment options are available.

Palilalia, a complex tic, is a language disorder characterized by the involuntary repetition of syllables, words, or phrases. It has features resembling other complex tics such as echolalia or coprolalia, but, unlike other aphasias, palilalia is based upon contextually correct speech.

Speech repetition occurs when individuals speak the sounds that they have heard another person pronounce or say. In other words, it is the saying by one individual of the spoken vocalizations made by another individual. Speech repetition requires the person repeating the utterance to have the ability to map the sounds that they hear from the other person's oral pronunciation to similar places and manners of articulation in their own vocal tract.

Imitative learning is a type of social learning whereby new behaviors are acquired via imitation. Imitation aids in communication, social interaction, and the ability to modulate one's emotions to account for the emotions of others, and is "essential for healthy sensorimotor development and social functioning". The ability to match one's actions to those observed in others occurs in humans and animals; imitative learning plays an important role in humans in cultural development. Imitative learning is different from observational learning in that it requires a duplication of the behaviour exhibited by the model, whereas observational learning can occur when the learner observes an unwanted behaviour and its subsequent consequences and as a result learns to avoid that behaviour.

Echophenomenon is "automatic imitative actions without explicit awareness" or pathological repetitions of external stimuli or activities, actions, sounds, or phrases, indicative of an underlying disorder.

The Motor tic, Obsessions and compulsions, Vocal tic Evaluation Survey (MOVES) is a psychological measure used to screen for tics and other behaviors. It measures "motor tics, vocal tics, obsessions, compulsions, and associated symptoms including echolalia, echopraxia, coprolalia, and copropraxia".

Palipraxia is a rare complex tic involving the involuntary repetition of one's own movements. It is associated with Tourette syndrome and may be associated with epilepsy.

A premonitory urge is a sensory phenomenon associated with Tourette syndrome and other tic disorders. Premonitory urges are "uncomfortable feelings or sensations preceding tics that usually are relieved by [a particular] movement".
