The Fire Fighter Fatality Investigation and Prevention Program (FFFIPP) is administered by the National Institute for Occupational Safety and Health (NIOSH), part of the Center for Disease Control and Prevention (CDC). It performs independent investigations of firefighter fatalities in the United States, also referred to as line of duty deaths (LODD).

In 1998, Congress funded NIOSH to implement FFFIPP recognizing the need for further efforts to address the continuing national problem of occupational fire fighter fatalities--an estimated 105 each year. [2] The NIOSH-FFFIPP has a 14-person staff and a $2 million budget [3] and has conducted over 400 investigations since its inception in 1998. [3]
In April 2014, Fire Engineering reported the release of a NIOSH map tracking firefighter deaths as part of the FFFIPP program. [4]
In August 2024, NIOSH put out a request for comment in the Federal Register [5] as to whether to include "human factors" in investigations. [6]
A firefighter is a first responder trained in firefighting, primarily to control and extinguish fires that threaten life and property, as well as to rescue persons from confinement or dangerous situations. Male firefighters are sometimes referred to as firemen.

Construction site safety is an aspect of construction-related activities concerned with protecting construction site workers and others from death, injury, disease or other health-related risks. Construction is an often hazardous, predominantly land-based activity where site workers may be exposed to various risks, some of which remain unrecognized. Site risks can include working at height, moving machinery and materials, power tools and electrical equipment, hazardous substances, plus the effects of excessive noise, dust and vibration. The leading causes of construction site fatalities are falls, electrocutions, crush injuries, and caught-between injuries.

This encompasses many types of hazards, including chemical hazards, biological hazards (biohazards), psychosocial hazards, and physical hazards. In the United States, the National Institute for Occupational Safety and Health (NIOSH) conduct workplace investigations and research addressing workplace health and safety hazards resulting in guidelines. The Occupational Safety and Health Administration (OSHA) establishes enforceable standards to prevent workplace injuries and illnesses. In the EU, a similar role is taken by EU-OSHA.
The Charleston Sofa Super Store fire disaster occurred on the evening of June 18, 2007, in Charleston, South Carolina, and killed nine firefighters. This was the deadliest firefighter disaster in the US since the September 11 attacks. Though the fire was believed to have started in some discarded furniture in the loading dock area, the exact source of ignition remains undetermined. After the fire, the store was demolished and bought by Charleston and a fire station was built nearby.
The Fire Fighter Near Miss Reporting System was launched on August 12, 2005 by the International Association of Fire Chiefs. It was announced at a press conference in Denver, Colorado, after having completed a pilot program involving 38 fire departments across the country. The Near Miss Reporting System aims to prevent injuries and save lives of other firefighters by collecting, sharing and analyzing near-miss experiences. The near-miss experiences are collected by firefighters who voluntarily submit them; the reports are confidential, non-punitive, and secure. After the reports are compiled, they are posted to the website where firefighters can access them and learn from each other's real-life experiences. Overall these reports help to formulate strategies, reduce firefighter injuries and fatalities, and enhance the safety culture of the fire service. The program is based on the Aviation Safety Reporting System (ASRS), which has been gathering reports of close calls from pilots, flight attendants, air traffic controllers since 1976. The reporting system is funded by the International Association of Fire Chiefs.
The Centers for Agricultural Safety and Health (CASH) are a set of 12 NIOSH-funded agencies focused on occupational health in industry involving food or plant products, such as fishing, forestry, and agriculture. The agencies were established in 1990 under the Agricultural Health and Safety Initiative.
Workplace health surveillance or occupational health surveillance (U.S.) is the ongoing systematic collection, analysis, and dissemination of exposure and health data on groups of workers. The Joint ILO/WHO Committee on Occupational Health at its 12th Session in 1995 defined an occupational health surveillance system as "a system which includes a functional capacity for data collection, analysis and dissemination linked to occupational health programmes".
Prevention through design (PtD), also called safety by design usually in Europe, is the concept of applying methods to minimize occupational hazards early in the design process, with an emphasis on optimizing employee health and safety throughout the life cycle of materials and processes. It is a concept and movement that encourages construction or product designers to "design out" health and safety risks during design development. The process also encourages the various stakeholders within a construction project to be collaborative and share the responsibilities of workers' safety evenly. The concept supports the view that along with quality, programme and cost; safety is determined during the design stage. It increases the cost-effectiveness of enhancements to occupational safety and health.

A physical hazard is an agent, factor or circumstance that can cause harm with contact. They can be classified as type of occupational hazard or environmental hazard. Physical hazards include ergonomic hazards, radiation, heat and cold stress, vibration hazards, and noise hazards. Engineering controls are often used to mitigate physical hazards.
A line of duty death (LODD) is a death in the fire service or the police service while on duty.
Fall protection is the use of controls designed to protect personnel from falling or in the event they do fall, to stop them without causing severe injury. Typically, fall protection is implemented when working at height, but may be relevant when working near any edge, such as near a pit or hole, or performing work on a steep surface. Many of these incidents are preventable when proper precautions are taken, making fall protection training not only critical, but also required for all construction workers. Occupational safety and health (OSH) or occupational health and safety (OHS) is a multidisciplinary field concerned with the safety, health, and welfare of people at work. OSH is related to the fields of occupational medicine and occupational hygiene and aligns with workplace health promotion initiatives. OSH also protects all the general public who may be affected by the occupational environment.
The Office of Mine Safety and Health Research (OMSHR) was a division within the United States' National Institute for Occupational Safety and Health (NIOSH) devoted towards the elimination of mining fatalities, injuries, and illnesses through research and prevention.
People who are driving as part of their work duties are an important road user category. First, workers themselves are at risk of road traffic injury. Contributing factors include fatigue and long work hours, delivery pressures, distractions from mobile phones and other devices, lack of training to operate the assigned vehicle, vehicle defects, use of prescription and non-prescription medications, medical conditions, and poor journey planning. Death, disability, or injury of a family wage earner due to road traffic injury, in addition to causing emotional pain and suffering, creates economic hardship for the injured worker and family members that may persist well beyond the event itself.
The Southwest Inn fire in Houston, Texas on May 31, 2013 was a fire in an Indian restaurant that spread to an adjoining hotel. The fire claimed the biggest casualty loss for the Houston Fire Department since its inception. Four firefighters were killed and 13 others were injured that day while fighting the five-alarm fire at the Southwest Inn located in Southwest Houston. On March 7, 2017, Captain Bill Dowling, who lost both his legs battling the fire in 2013, died of complications from his line-of-duty injuries.
The National Farm Medicine Center (NFMC), established in 1981, is a non-profit program in Marshfield, Wisconsin dedicated to agricultural health and safety research, and service. The center is a component of the Marshfield Clinic Research Institute, which is part of Marshfield Clinic.
