Anatomical terminology

The tracheobronchial lymph nodes are lymph nodes that are located around the division of trachea and main bronchi.

Structure

These lymph nodes form four main groups including paratracheal, tracheobronchial, bronchopulmonary and pulmonary nodes.

  1. Paratracheal nodes are located on either side of the trachea.
  2. Tracheobronchial nodes can be divided into three nodes including left and right superior tracheobronchial nodes, and the inferior trachiobronchial node. The two superior tracheobronchial nodes are located on either side of trachea just before its bifurcation. The inferior tracheobronchial node is located just below the bifurcation in the angle between the two bronchi.
  3. Bronchopulmonary nodes (hilar nodes) situate in the hilum of each lung.
  4. Pulmonary nodes are embedded the lung substance on the larger branches of the bronchi.

The afferents of the tracheobronchial glands drain the lungs and bronchi, the thoracic part of the trachea and the heart; some of the efferents of the posterior mediastinal glands also end in this group.

Their efferent vessels ascend upon the trachea and unite with efferents of the internal mammary and anterior mediastinal glands to form the right and left bronchomediastinal trunks.

The trachea, also known as the windpipe, is a cartilaginous tube that connects the larynx to the bronchi of the lungs, allowing the passage of air, and so is present in almost all animals with lungs. The trachea extends from the larynx and branches into the two primary bronchi. At the top of the trachea the cricoid cartilage attaches it to the larynx. The trachea is formed by a number of horseshoe-shaped rings, joined together vertically by overlying ligaments, and by the trachealis muscle at their ends. The epiglottis closes the opening to the larynx during swallowing.

A bronchus is a passage or airway in the lower respiratory tract that conducts air into the lungs. The first or primary bronchi to branch from the trachea at the carina are the right main bronchus and the left main bronchus. These are the widest bronchi, and enter the right lung, and the left lung at each hilum. The main bronchi branch into narrower secondary bronchi or lobar bronchi, and these branch into narrower tertiary bronchi or segmental bronchi. Further divisions of the segmental bronchi are known as 4th order, 5th order, and 6th order segmental bronchi, or grouped together as subsegmental bronchi. The bronchi, when too narrow to be supported by cartilage, are known as bronchioles. No gas exchange takes place in the bronchi.

The bronchioles or bronchioli are the smaller branches of the bronchial airways in the lower respiratory tract. The bronchioles no longer contain the cartilage that is found in the bronchi, or glands in their submucosa.

The root of the lung is a group of structures that emerge at the hilum of each lung, just above the middle of the mediastinal surface and behind the cardiac impression of the lung. It is nearer to the back than the front. The root of the lung is connected by the structures that form it to the heart and the trachea. The rib cage is separated from the lung by a two-layered membranous coating, the pleura. The hilum is the large triangular depression where the connection between the parietal pleura and the visceral pleura is made, and this marks the meeting point between the mediastinum and the pleural cavities.

A bronchopulmonary segment is a portion of lung supplied by a specific segmental bronchus and its vessels. These arteries branch from the pulmonary and bronchial arteries, and run together through the center of the segment. Veins and lymphatic vessels drain along the edges of the segment. The segments are separated from each other by layers of connective tissue that forms them into discrete anatomical and functional units. This separation means that a bronchopulmonary segment can be surgically removed without affecting the function of the others.

The eparterial bronchus is a branch of the right main bronchus given off about 2.5 cm from the bifurcation of the trachea. This branch supplies the superior lobe of the right lung and is the most superior of all secondary bronchi. It arises above the level of the right pulmonary artery, and for this reason is named the eparterial bronchus. All other distributions falling below the pulmonary artery are termed hyparterial.

Lung cancer staging is the assessment of the extent to which a lung cancer has spread from its original source. As with most cancers, staging is an important determinant of treatment and prognosis. In general, more advanced stages of cancer are less amenable to treatment and have a worse prognosis.

The Intrapulmonary nodes or Lymphatic Vessels of the Lungs originate in two plexuses, a superficial and a deep. The superficial plexus is placed beneath the pulmonary pleura. The deep accompanies the branches of the pulmonary vessels and the ramifications of the bronchi. In the case of the larger bronchi the deep plexus consists of two networks—one, submucous, beneath the mucous membrane, and another, peribronchial, outside the walls of the bronchi. In the smaller bronchi there is but a single plexus, which extends as far as the bronchioles, but fails to reach the alveoli, in the walls of which there are no traces of lymphatic vessels. The superficial efferents turn around the borders of the lungs and the margins of their fissures, and converge to end in some glands situated at the hilus; the deep efferents are conducted to the hilus along the pulmonary vessels and bronchi, and end in the tracheobronchial lymph nodes. Little or no anastomosis occurs between the superficial and deep lymphatics of the lungs, except in the region of the hilus. they are located in right fissure of lung near the heart

The superior diaphragmatic lymph nodes lie on the thoracic aspect of the diaphragm, and consist of three sets – anterior, middle, and posterior.
