The spinal root of accessory nerve (or part) is firm in texture, and its fibers arise from the motor cells in the lateral part of the anterior column of the gray substance of the medulla spinalis as low as the fifth cervical nerve.
Passing through the lateral funiculus of the medulla spinalis, they emerge on its surface and unite to form a single trunk, which ascends between the ligamentum denticulatum and the posterior roots of the spinal nerves; enters the skull through the foramen magnum, and is then directed to the jugular foramen, through which it passes, lying in the same sheath of dura mater as the vagus, but separated from it by a fold of the arachnoid.
The most lateral of the bundles of the anterior nerve roots is generally taken as a dividing line that separates the anterolateral system into two parts. These are the anterior funiculus, between the anterior median fissure and the most lateral of the anterior nerve roots, and the lateral funiculus between the exit of these roots and the posterolateral sulcus.
The jugular foramen is a large foramen (aperture) in the base of the skull. It is located behind the carotid canal and is formed in front by the petrous portion of the temporal bone, and behind by the occipital bone; it is generally larger on the right than on the left side.
Dura mater is a thick membrane made of dense irregular connective tissue that surrounds the brain and spinal cord. It is the outermost of the three layers of membrane called the meninges that protect the central nervous system. The other two meningeal layers are the arachnoid mater and the pia mater. The dura surrounds the brain and the spinal cord and is responsible for keeping in the cerebrospinal fluid. It is derived from neural crest cells.
In the jugular foramen, it receives one or two filaments from the cranial part of the nerve, or else joins it for a short distance and then separates from it again.
As it exits from the jugular foramen, it runs backward in front of the internal jugular vein in 66.6% of cases, and behind it in 33% of cases.
The internal jugular vein is a paired jugular vein that collects blood from the brain and the superficial parts of the face and neck. The vein runs in the carotid sheath with the common carotid artery and vagus nerve.
The nerve then descends obliquely behind the Digastricus and Stylohyoideus to the upper part of the Sternocleidomastoideus; it pierces this muscle, and courses obliquely across the posterior triangle of the neck, to end in the deep surface of the Trapezius.
As it traverses the Sternocleidomastoideus it gives several filaments to the muscle, and joins with branches from the second cervical nerve.
In the posterior triangle it unites with the second and third cervical nerves, while beneath the Trapezius it forms a plexus with the third and fourth cervical nerves, and from this plexus fibers are distributed to the muscle.
A spinal nerve is a mixed nerve, which carries motor, sensory, and autonomic signals between the spinal cord and the body. In the human body there are 31 pairs of spinal nerves, one on each side of the vertebral column. These are grouped into the corresponding cervical, thoracic, lumbar, sacral and coccygeal regions of the spine. There are eight pairs of cervical nerves, twelve pairs of thoracic nerves, five pairs of lumbar nerves, five pairs of sacral nerves, and one pair of coccygeal nerves. The spinal nerves are part of the peripheral nervous system.
The glossopharyngeal nerve, known as the ninth cranial nerve, is a mixed nerve that carries afferent sensory and efferent motor information. It exits the brainstem out from the sides of the upper medulla, just rostral to the vagus nerve. The motor division of the glossopharyngeal nerve is derived from the basal plate of the embryonic medulla oblongata, while the sensory division originates from the cranial neural crest.
The accessory nerve is a cranial nerve that supplies the sternocleidomastoid and trapezius muscles. It is considered the eleventh of twelve pairs of cranial nerves, or simply cranial nerve XI, as part of it was formerly believed to originate in the brain. The sternocleidomastoid muscle tilts and rotates the head, while the trapezius muscle, connecting to the scapula, acts to shrug the shoulder.
The great auricular nerve originates from the cervical plexus, composed of branches of spinal nerves C2 and C3. It provides sensory innervation for the skin over parotid gland and mastoid process, and both surfaces of the outer ear.
In human anatomy, the subclavian arteries are paired major arteries of the upper thorax, below the clavicle. They receive blood from the aortic arch. The left subclavian artery supplies blood to the left arm and the right subclavian artery supplies blood to the right arm, with some branches supplying the head and thorax. On the left side of the body, the subclavian comes directly off the aortic arch, while on the right side it arises from the relatively short brachiocephalic artery when it bifurcates into the subclavian and the right common carotid artery.
The sternocleidomastoid muscle is one of the largest and most superficial cervical muscles. The primary actions of the muscle are rotation of the head to the opposite side and flexion of the neck. The sternocleidomastoid is innervated by the accessory nerve.
The levator scapulae is a skeletal muscle situated at the back and side of the neck. As the Latin name suggests, its main function is to lift the scapula.
The vertebral arteries are major arteries of the neck. Typically, the vertebral arteries originate from the subclavian arteries. Each vessel courses superiorly along each side of the neck, merging within the skull to form the single, midline basilar artery. As the supplying component of the vertebrobasilar vascular system, the vertebral arteries provide supply blood to the upper spinal cord, brainstem, cerebellum, and posterior part of brain.
The dorsal root of spinal nerve is one of two "roots" which emerge from the spinal cord. It emerges directly from the spinal cord, and travels to the dorsal root ganglion. Nerve fibres with the ventral root then combine to form a spinal nerve. The dorsal root transmits sensory information, forming the afferent sensory root of a spinal nerve.
The splenius capitis is a broad, straplike muscle in the back of the neck. It pulls on the base of the skull from the vertebrae in the neck and upper thorax. It is involved in movements such as shaking the head.
The occipital artery arises from the external carotid artery opposite the facial artery. Its path is below the posterior belly of digastric to the occipital region. This artery supplies blood to the back of the scalp and sterno-mastoid muscles, and deep muscles in the back and neck.
The posterior triangle is a region of the neck.
The supraclavicular nerves arise from the third and fourth cervical nerves; they emerge beneath the posterior border of the sternocleidomastoideus, and descend in the posterior triangle of the neck beneath the platysma and deep cervical fascia.
The transverse cervical nerve arises from the second and third spinal nerves, turns around the posterior border of the sternocleidomastoideus about its middle, and, passing obliquely forward beneath the external jugular vein to the anterior border of the muscle, it perforates the deep cervical fascia, and divides beneath the Platysma into ascending and descending branches, which are distributed to the antero-lateral parts of the neck. It provides cutaneous innervation to this area.
The carotid triangle is a portion of the anterior triangle of the neck.
The subclavian triangle, the smaller division of the posterior triangle, is bounded, above, by the inferior belly of the omohyoideus; below, by the clavicle; its base is formed by the posterior border of the sternocleidomastoideus.
The occipital triangle, the larger division of the posterior triangle, is bounded, in front, by the Sternocleidomastoideus; behind, by the Trapezius; below, by the Omohyoideus.
The posterior branches of cervical nerves branch from the dorsal rami of the cervical nerves.

The cranial root of accessory nerve is the smaller of the two portions of the accessory nerve. It is generally considered as a part of the vagus nerve and not part of the accessory nerve proper because the cranial component rapidly joins the vagus nerve and serves the same function as other vagal nerve fibers. Recently, the concept of a cranial root of the accessory nerve has been challenged by new neuroanatomical studies which found that an unambiguous cranial root was not present in the majority of the cases. However, a small study in 2007 followed by a substantially larger study published in 2012 both confirmed that the cranial root of the accessory nerve is commonly found in humans, matching traditional descriptions.
