Suxethonium (trade name: Brevidil-E) is a depolarising muscle relaxant which is presented as a dry powder in an ampoule. This is re-constituted with sterile water prior to use. It is available in Australia as a schedule 4 drug, [1] and the US. [2]

It has some advantages over suxamethonium:

The last advantage was what keeps it in occasional use.

Previous use

In the UK, it was used by the Obstetric Flying Squads at patient's homes to resuscitate mothers during major obstetric complications (mostly major haemorrhage) during home births.

In Australia, it was included in "Resuscitation drugs packs" in hospitals. These packs were sealed boxes containing all the drugs required for an in-hospital resuscitation. They were prepared by the hospital pharmacy and because these were sealed (usually just sticky tape) the drug contents were guaranteed to be there for use in a cardiac arrest. These packs contained ampoules of powdered suxethonium so a relaxant was available to facilitate intubation. Suxamethonium could not be used in these packs because of the requirement for refrigeration. This was certainly an issue in Queensland as it could be quite warm and hospitals wards were in the past generally not air-conditioned.

It can still be used where the storage issue is a concern (e.g. by the military, rural locations, 3rd world countries). The most recent reference to it in Medline is 1976, but it is occasionally mentioned in passing in other journals. [4]

Curare is a common name for various alkaloid arrow poisons originating from plant extracts. Used as a paralyzing agent by indigenous peoples in Central and South America for hunting and for therapeutic purposes, curare only becomes active when it contaminates a wound. Candocuronium iodide is an aminosteroid neuromuscular-blocking drug. Its use in anesthesia for endotracheal intubation and to provide skeletal muscle relaxation during surgery or mechanical ventilation was briefly evaluated in clinical studies in India. However, further development was discontinued due to attendant cardiovascular effects, primarily tachycardia that was about the same as the clinically established pancuronium bromide. Candocuronium demonstrated a short duration in the body, but a rapid onset of action. It had little to no ganglion blocking activity, with a greater potency than pancuronium.

<span class="mw-page-title-main">Coinduction (anesthetics)</span>

Coinduction in anesthesia is a pharmacological tool whereby a combination of sedative drugs may be used to greater effect than a single agent, achieving a smoother onset of general anesthesia. The use of coinduction allows lower doses of the same anesthetic agents to be used which provides enhanced safety, faster recovery, fewer side-effects, and more predictable pharmacodynamics. Coinduction is used in human medicine and veterinary medicine as standard practice to provide optimum anesthetic induction. The onset or induction phase of anesthesia is a critical period involving the loss of consciousness and reactivity in the patient, and is arguably the most dangerous period of a general anesthetic. A great variety of coinduction combinations are in use and selection is dependent on the patient's age and health, the specific situation, and the indication for anesthesia. As with all forms of anesthesia the resources available in the environment are a key factor.
