The highway extends for 9.84 miles (15.84 km) in a northeast–southwest direction from an intersection with NY 79 south of the hamlet of Reynoldsville in the town of Hector to a junction with NY 96 in the village of Trumansburg. In between, NY 227 intersects NY 228 in Perry City by the east town line of Hector and passes through the town of Ulysses. Part of NY 227 straddles the Schuyler–Tompkins county line.

In Indiana, it is a state road that enters the state in College Corner, Ohio, and West College Corner. The 117.765 miles (189.524 km) of US 27 that lie within Indiana serve as a major conduit. Most of the highway is listed on the National Highway System. Various sections are a rural two-lane highway or an urbanized four-lane divided highway. The northernmost community along the highway is Fort Wayne in the northeast part of the state. The highway ends at an interchange with Interstate 69 (I-69) north of downtown Fort Wayne after serving the east central and northeast regions of Indiana.

Maryland Route 227 is a state highway in the U.S. state of Maryland. The state highway runs 13.96 miles (22.47 km) from Marshall Hall east to U.S. Route 301 in White Plains. MD 227 connects the communities of Bryans Road, Pomonkey, and Pomfret in northwestern Charles County. The state highway, which was constructed in the mid-1920s and early 1930s, originally had its western terminus at Pomonkey; the remainder of the current route was part of MD 224 and all of MD 226. MD 227 gained its present western terminus in the mid-1950s.
