The Boksburg explosion took place on 24 December 2022, when a fuel tanker carrying liquefied petroleum gas (LPG) exploded underneath a railway bridge in Boksburg, in the Ekurhuleni Metropolitan Municipality in Gauteng, South Africa, [1] with a death toll of 41 people as of 18 January 2023. [2] Nearby infrastructure was damaged by the explosion.
The truck belonged to Infinite Transport, part of the Sasolburg-based Infinite Group, and was carrying 60,000 litres (15,850 gal) of LPG from the Port of Richards Bay to Botswana. [3] [4] [5] The driver was employed by Innovative Staffing Solutions (ISS), a Boksburg-based outsourcing company that provides staff on contract to other companies. [3] [6] [7]
According to his employer, on the morning of 24 December 2022, having spent the night at a nearby truck stop, the 32 year-old driver took a wrong turn [7] [8] and tried to return to his route; the route he used took him under a railway bridge spanning Hospital Street near its intersection with Railway Street, about 100 metres (330 ft) from the Tambo Memorial Hospital in the Boksburg suburb of Plantation. [7] [9] [4]
According to the driver's employer, ISS, the bridge appeared to be high enough for the truck to pass, but the incline at the bottom of the bridge raised the back of the truck and jammed it against the underside of the bridge at approximately 06:15am SAST, [9] and when the driver realised the truck had become stuck, he stopped immediately to assess the damage. According to ISS, upon noticing that the tanker's cap had been scraped off, he first informed them and then alerted the fire department, then attempted to cordon off the area. This met with limited success despite assistance from a private security guard who happened to drive onto the scene, as well as two bystanders who realised the potential danger. [9] [10] Investigative news programme Carte Blanche found that the calls to emergency services were initially unsuccessful, and that the Ekurhuleni Fire Department eventually answered a call. Carte Blanche also found that law enforcement (SAPS and the Metro Police) didn't arrive at the scene until after the final explosion. [7] Carte Blanche also found that the height of the truck was 4m, higher than the 3.6m indicated on the bridge, potentially contradicting the account of his employer. [7]
At about 06:45am SAST, the truck ignited. Having caught fire, it burst in a massive boiling liquid expanding vapor explosion (BLEVE) at approximately 07:05 am SAST while firefighters were still trying to contain the blaze. [9] [11] According to his employer, the driver had been overcome by gas inhalation before this point, and had been taken to hospital by emergency workers. [9] The initial explosion was quickly followed by two smaller explosions. [12] This series of explosions attracted a large crowd of onlookers, resulting in a higher number of casualties and fatalities when a fourth explosion – the largest, and last – occurred roughly around 07:30am SAST. [13] [14] The final explosion affected buildings up to 400 metres (1,300 ft) away, and was felt as an earth tremor as far as 4 kilometres (2.5 mi) away. [15]
The explosion resulted in a high number of deaths and injuries and extensive damage to property and infrastructure. Damage to the Tambo Memorial Hospital left it unable to render assistance in the immediate aftermath, despite its proximity to the disaster site.
41 people were killed by the explosion. [2] 40 people were initially injured and hospitalised after the explosion, many of whom later succumbed to their wounds. [1] [16] Eight people were killed immediately during the blast, or else succumbed before paramedics arrived on the scene, while the other victims died at hospital. [17] Two people who were in the vicinity of the explosion are still missing as of 29 December 2022. [18] The victims were identified as 23 members of the public and 11 health workers of the damaged hospital. [19]
The hospital adjacent to the explosion was severely impacted. Eight staff members of the hospital (seven nurses and a driver) were killed in the carpark of the hospital, [20] and 24 patients and 13 staff members, present in the hospital's emergency unit at the time of the explosion, sustained severe burns and were transferred to other hospitals nearby. [21] [ needs update ] At least one of those killed in the hospital carpark was attempting to move their car away from the burning tanker. [22] The explosion resulted in the ceiling of the emergency unit partially collapsing which necessitated all patients being moved to the theatre complex at the back of the hospital. The hospital was unable to accept patients immediately after the blast. [23] Engineers who assessed the structure later found that the damage was limited, and the hospital remained usable. [24] In an interview with SABC News, the hospital CEO stated that it was fully operational by the end of January 2023. [25]
Two houses and a number of cars were destroyed in the explosion, as well as a fire truck that was attempting to extinguish the growing fire. [26] Properties in the area were severely damaged by the fires and explosions, and residents who were in the area to assist were killed. [27] The explosion also destroyed the railway bridge underneath which it occurred, affecting the Germiston–Springs railway line. The Passenger Rail Agency of South Africa (Prasa) announced the following day that it would assess the extent of the damage, and the scope of the work necessary to restore the railway line. [28]
The initial response to the incident came from local security companies, the local Community Policing Forum (CPF) and volunteer groups. On being alerted to the leaking gas tanker by the driver, the alarm was raised and Ekurhuleni Disaster & Emergency Management Services dispatched a fire truck from Boksburg Central fire station. A number of non-governmental organisations (NGOs), including Muslim Aid and the South African disaster relief NGO Gift of the Givers, arrived to assist throughout the day.
A secondary, medical evacuation operation to urgently move critical patients from the Tambo Memorial Hospital to other hospitals for further or palliative care was coordinated by the Gauteng Emergency Medical Services (Gauteng EMS), assisted by private ambulance services and volunteer groups. Lower priority patients were moved in the second phase of the evacuation. A total of 30 state and private ambulances as well as several air ambulance helicopters transferred the patients over a three-hour period.
The driver of the tanker survived the incident and initially faced multiple charges, including culpable homicide, which were later dropped due to lack of evidence. [29] [30] The driver remained un-charged six months after the explosion, and was reportedly transporting hazardous materials for his employer. [31]
The day after the incident, the South African Police Service (SAPS) stated that the driver, who received only minor injuries during the incident, had been arrested at a local hospital and would face charges of reckless driving, negligence, and multiple counts of culpable homicide and malicious damage to property. [4] [21] He was expected to appear before the Boksburg Magistrate's Court on 28 December 2022; however, on that day it was reported that he had been released and the charges against him had been dropped, as there was not enough evidence against him. [32] [33] [34]
Gauteng premier Panyaza Lesufi expressed disappointment at the dropping of charges against the driver, and asked the police and National Prosecuting Authority (NPA) to swiftly re-instate them. He told the press that he had been re-assured that police and prosecutors were still building a stronger case against the driver, against whom he suggested harsher charges would be instated by 30 December 2022. [35] According to Lesufi, the original charges were not enrolled in court because the NPA felt more evidence was needed before proceeding. [35] However, Lesufi's claims were contradicted by Henk Strydom, the senior state prosecutor at Boksburg Magistrate's Court, who said that the decision to release the driver was taken solely by the SAPS, and not in consultation with the NPA. [36]
The deputy provincial police commissioner for Gauteng, General Mbuso Khumalo, stated on the day of the driver's release that his initial arrest was in fact unlawful, because it was not preceded by an investigation into the driver's intent. According to Khumalo, an investigation must be conduced before an arrest can be made in South Africa. [36] However, Khumalo also stated that the driver was not necessarily "out of the woods", and that a large police team was conducting a thorough investigation. [36]
On 30 December 2022, national police spokesperson Brenda Muridili announced that the police investigation of the explosion was considered "top priority" by the provincial SAPS in Gauteng, and stated that the investigation was at an advanced stage. [37]
The Citizen reported, based on Google Street View imagery, that a height restriction warning sign on the bridge was illegible. [38] [ dubious – discuss ] [3] The Citizen report led to Department of Transport, which is responsible for Prasa, and the railway sign, and the municipality, blaming each other. [3] A municipal report later stated that the sign was legible and photos of the area taken during recent flooding showed that it had been affixed to the bridge prior to the explosion. [39]
Amid claims of potential municipal or parastatal negligence, there were calls for a thorough investigation to determine who was, in fact, responsible for the disaster. [3]
ISS, the company that sourced the driver on behalf of Infinite Transport, began an "internal and independent" investigation into the circumstances surrounding the explosion, which concluded on 29 December. It stated the driver had seven years of experience transporting hazardous substances, and that he "was not negligent and did everything in his power to alert people to the dangers of the situation". [3] [9] The company also said it would hand over the findings of its investigation to the authorities. The conclusions of the internal investigation were questioned by a former employee of the company that owned the truck. [40]
Another report, based on documentation provided to an external auditor [41] commissioned by the owner of the truck, Infinite Transport, stated that the company was compliant with its legal obligations, [39] however the conclusions of this report were dismissed by NUMSA leader Irvin Jim as a "ploy" by the trucking company to evade responsibility. [41]
An SABC news report in late May 2023 stated that the driver had been "cleared" of wrongdoing. [25] A later report, in June 2023, indicated that prosecutors were deciding whether to charge the driver. [42]
Various government officials and South African political parties have reacted to the explosion with condolences to the families of the victims and well-wishes toward the injured, and in some cases calls for changes in response to the disaster. A number of Middle Eastern and North African countries have also reacted through official channels, generally with sentiments of solidarity toward the South African people and government.
Benoni is a town on the East Rand, Gauteng, South Africa, some 26 kilometres (16 mi) east of Johannesburg, within the City of Ekurhuleni municipality.
The East Rand is a major urban area located in the Gauteng province of South Africa. It is the urban eastern part of Witwatersrand that is functionally merged with the Johannesburg conurbation. The region extends from Alberton in the west to Springs in the east, and south down to Nigel. It includes the towns of Bedfordview, Benoni, Boksburg, Brakpan, Edenvale, Germiston, Kempton Park, Linksfield and Modderfontein. The East Rand is known as the transport hub of Johannesburg and includes Africa's largest and second busiest airport, OR Tambo International Airport.
Boksburg is a city on the East Rand of Gauteng province of South Africa. Gold was discovered in Boksburg in 1887. Boksburg was named after the State Secretary of the South African Republic, W. Eduard Bok. The Main Reef Road linked Boksburg to all the other major mining towns on the Witwatersrand and the Angelo Hotel (1887) was used as a staging post.
Kempton Park is a city in the East Rand region of Gauteng province, South Africa. It is part of the City of Ekurhuleni Metropolitan Municipality. It is situated south of Thembisa, one of the largest townships in South Africa, which is also part of Ekurhuleni. South Africa's busiest airport, O. R. Tambo International Airport is located in Kempton Park.
The R21 is a major north–south provincial route in eastern Gauteng Province, South Africa. Built in the early 1970s, it remains one of two freeways linking Pretoria with Johannesburg, via the R24. As the eastern of the two freeways, it links the Pretoria city centre with OR Tambo International Airport and Boksburg. Between the Solomon Mahlangu Drive on-ramp in Monument Park, Pretoria, and the N12 interchange in Boksburg, the R21 is an 8 lane highway and motorway (freeway), with 4 lanes in each direction. It has off-ramps leading to Irene, Olifantsfontein, Benoni, and Kempton Park. The route intersects the N1 highway near Centurion, the R24 near the airport, the N12 and N17 in Boksburg, and the N3 near Vosloorus on the East Rand, where it ends. The section from the N12 to the N3 is not a freeway. The R21 is also designated as the P157.
The R24 is a major East-West provincial route in the Gauteng and North West provinces that links OR Tambo International Airport with Rustenburg via Johannesburg, Krugersdorp and Magaliesburg. The section of the route from OR Tambo International westwards up to a point in Roodepoort is has been named Albertina Sisulu Road since 2013, named after anti-apartheid stalwart Albertina Sisulu.
A gas explosion is the ignition of a mixture of air and flammable gas, typically from a gas leak. In household accidents, the principal explosive gases are those used for heating or cooking purposes such as natural gas, methane, propane, butane. In industrial explosions many other gases, like hydrogen, as well as evaporated (gaseous) gasoline or ethanol play an important role. Industrial gas explosions can be prevented with the use of intrinsic safety barriers to prevent ignition, or use of alternative energy.
The 2005 Delhi bombings occurred on 29 October 2005 in Delhi, India, killing 62 people and injuring at least 210 others in three explosions. The bombings came only two days before the important festival of Diwali, which is celebrated by Hindus, Sikhs, and Jains. The bombs were triggered in two markets in central and south Delhi and in a bus south of the city. The Pakistani Islamist terrorist group Lashkar-e-Taiba claimed responsibility for the attacks under the pseudonym of Islamic Inquilab Mahaz. The Indian Mujahideen is also suspected of involvement.
The City of Ekurhuleni Metropolitan Municipality is a metropolitan municipality that forms the local government of the East Rand region of Gauteng, a large suburban region east of Johannesburg. Ekurhuleni means "place of peace" in XiTsonga. Ekurhuleni is one of the five districts of Gauteng and one of the eight metropolitan municipalities of South Africa. The seat of Ekurhuleni is Germiston and the most spoken language is Zulu at 28.6%. The city is home to South Africa's busiest airport, OR Tambo International Airport, which is in the Kempton Park area of Ekurhuleni.
On 14 October 2017, two truck bombings took place in Mogadishu, the capital of Somalia, killing at least 587 people and injuring 316 others. Almost all of the casualties were caused by one of the trucks which detonated when the driver, while attempting to escape from security officials, crashed through a barrier and exploded in the Hodan District, destroying a hotel. The intended target of the attack is believed to have been a secure compound housing international agencies and troops. The second blast happened close by, killing two people. A third explosives-laden truck was captured by police.

He previously served as Executive Mayor of the Ekurhuleni Metropolitan Municipality (2010–2016), as Deputy Minister of Finance (2018–2019), as Chairperson of the Social Development Committee (2019–2021) and as Minister in the Presidency (2021–2023).
On December 23, 1988, a tractor-trailer tanker truck hauling liquefied propane crashed on an exit ramp at the Interstate 40/Interstate 240 (I-40/I-240) interchange in downtown Memphis, Tennessee, US. The crash ruptured the tank; the leaking gas exploded, setting multiple vehicles and structures on fire. The explosion propelled the tank 125 yards (114 m) into a residential complex and started additional fires. The accident caused nine deaths and ten injuries. The interchange where the accident occurred was considered unsafe and poorly designed, and had been the site of several previous accidents; it was completely rebuilt in the 2000s.
On 14 December 2021, a fuel tank truck exploded in the Samari neighborhood of Cap-Haïtien, the capital city of the Haitian department of Nord. At least 90 people were killed and more than 120 were injured; many people were injured as a result of rushing towards the tanker, likely to collect some of its cargo, before the explosion occurred. Many inhabitants were suffering from financial crisis.
On 20 January 2022, a large explosion occurred along the Tarkwa-Bogoso-Ayamfuri road in Western Region, Ghana, after a truck transporting mining explosives collided with a motorcycle. The explosion levelled the nearby Apiate village, killing 13 people and injuring 200.
On 10 December 2022, an explosion destroyed a block of flats in St Helier, Jersey in the Channel Islands. Ten people were killed in the suspected gas explosion, which occurred just before 4 a.m. GMT.
The Tambo Memorial Hospital is a regional hospital located in Boksburg (Ekurhuleni) in Gauteng Province, South Africa.
On 5 July 2023, a gas leak killed 17 people, including three children, in Boksburg, Ekurhuleni, Gauteng, South Africa. Ten other people were hospitalised.
On 18 December 2023, an explosion and fire broke out at an oil depot in Conakry, Guinea, killing at least 24 people and injuring 454 and resulting in fuel shortages across the country.
The severe nature of the burns inflicted by the blast was expected to result in an increase in the number of deaths, authorities had earlier said.
Speaking outside court, Peter "Pocket" Mofokeng said he drove that exact tanker for a year until September, when he and other drivers were dismissed. "We had a dispute with the company over the use of a labour broker. This driver is among people brought in to work for the company through a labour broker," he said. "I've seen him drive small trucks. I've never seen him drive this truck. If he was experienced, he would have seen the bridge was low."
